You will receive an email with a copy of your form. Make sure to let our salon staff know you have filled out your form online when you arrive at your appointment.

On the day of your appointment:

• Bring valid identification to your appointment
• If your child (under 16) is being pierced please bring valid identification or other proof of guardianship
• Arrive 10 minutes before your scheduled appointment to allow time for consultation
